A triage and care-coordination aid for a nurse/care coordinator or an LLM triage layer, spanning multiple kidney-related disease and workflow modules. A single patient may activate several modules at once (e.g., syncope → Falls + Fluid + Arrhythmia).
It routes and gathers — it does not diagnose, prescribe, or change dialysis/medication orders. Classification and confidence have provisional suggestions with human override; thresholds are review triggers, not treatment rules. "Unable to determine" is a valid output.
Do not enter patient identifiers (PHI). This demonstration keeps all data in-memory only and stores nothing after the tab closes.
